What you'll be doing
The primary purpose of the position is to better facilitate the provision of co-ordinated and timely care for people with diabetes and to integrate existing diabetes services within the hospital, outpatients, specialists and General practitioners to create a "whole of district" approach.
- Five years post graduate experience including three years in speciality and recognised post graduate qualifications in diabetes education and management .
- Proven clinical leadership skills and ability to work collaboratively within multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
- Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based practice.
- Demonstrated experience and expert skills in the identification and management of diabetes, in the development and delivery of patient education and in the organisation and management of health professional training in diabetes.
