We are seeking an experienced and motivated Rural Generalist Nurse Practitioner (Hospital in the Home) to join our integrated care team within the Mid North Coast Local Health District.
This is a unique opportunity to work in a progressive, future‑focused service delivering high‑quality, person‑centred acute care in patients’ homes and community settings across the Mid North Coast. The Rural Generalist Nurse Practitioner will deliver complex, hospital‑level care within the Hospital in the Home (HITH) model, supporting admission avoidance, early discharge, and continuity of care.
The Nurse Practitioner will provide care primarily via face‑to‑face clinical delivery, ensuring timely and appropriate interventions are delivered in the most suitable setting across MNCLHD.
Grounded in nursing values, knowledge, and practice, the Nurse Practitioner role offers innovative and flexible healthcare delivery that complements and enhances multidisciplinary care. The scope of practice is defined by the authorised clinical context, enabling responsive, high‑quality care tailored to the needs of people living in rural and regional communities.
What you'll be doing
The Rural Generalist Nurse Practitioner will deliver high‑level, clinically focused care within the Hospital in the Home service, providing hospital‑level assessment, treatment, monitoring, and escalation of care as required.
Operating within a site‑specific model, the Nurse Practitioner will work autonomously and collaboratively with multidisciplinary teams to ensure patients receive safe, effective care in their home or community environment.
Key responsibilities include:
- Providing holistic, hospital‑level care for patients with acute and complex conditions in home and community settings.
- Working autonomously within an advanced nursing role while collaborating effectively with medical, nursing, and allied health teams.
- Supporting admission avoidance and early discharge through advanced clinical decision‑making and care planning.
- Making and receiving referrals to facilitate integrated care across health services and disciplines.
- Coordinating healthcare activities and liaising with internal and external community agencies and service providers.
- Supporting clinical escalation, patient safety, and continuity of care within the HITH model.
- Contributing to clinical excellence through leadership, education, consultation, research, and professional development.
- Creating culturally safe partnerships with priority populations, including Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse communities, people living with disability, and those experiencing social disadvantage.
