While you will have the opportunity to work independently, you will always have the strong support of your colleagues.

About the Role:

We have a permanent part-time position available, working Mondays and Tuesdays, with the flexibility to take on additional shifts as needed. This role offers the opportunity to travel around the beautiful Lake Macquarie region, providing a variety of clinical care services in clients' homes. If you love being on the road and making a difference, this could be the perfect fit for you!

To ensure a smooth transition, the successful applicant will undergo an orientation period of up to 4–6 weeks, gaining a clear understanding of documentation requirements and the team’s models of practice.

A dedicated mentor will provide guidance and support, alongside input from the NUM, CNS, and Nurse Practitioners, particularly in wound management and continence care.

We offer a structured orientation pathway for community nursing, with key education milestones at one month, three months, and 12 months to support your professional growth.

We are looking for someone who possesses:

  • Strong independence and teamwork skills – The ability to work autonomously while also collaborating effectively with colleagues, clients, and external service providers to ensure seamless and high-quality care.
  • Competency in wound care management – A foundational understanding of wound assessment, treatment, and ongoing management to promote healing and prevent complications.
  • Experience in continence management – The ability to assess, plan, and implement continence care strategies to support clients in maintaining dignity and comfort.
  • Proficiency in medication management – Ensuring safe administration, monitoring, and documentation of medications in accordance with best practices and client care plans.

We seek a compassionate, dedicated professional with excellent communication and problem-solving skills who is committed to delivering high-quality community care.
