Where you'll be working

This role is based within the WSLHD Residential Aged Care Assessment Service, which operates across residential aged care facilities in the Western Sydney Local Health District. The service delivers shadow assessments of residential aged care recipients using the Australian National Aged Care Classification (AN-ACC) tool, under a Commonwealth contract. 

       The AN-ACC Assessment Service is a WSLHD-administered service operating in accordance with Commonwealth requirements and performance targets.

       The role liaises with the Ministry of Health, other Local Health Districts and participates in a Clinical Community of Practice for AN-ACC Clinical Leads across NSW.

       Staff have access to salary packaging and the Employee Assistance Program (EAP).

       A valid unrestricted driver's licence is required, as the role involves travel to residential aged care facilities across the district.

 

What you'll be doing

The AN-ACC Team Leader is responsible for providing day-to-day oversight and clinical management of the AN-ACC Assessment Service, ensuring the service meets all Commonwealth requirements and performance targets. Key responsibilities will be to:

       Manage and coordinate the AN-ACC Assessment Service, providing leadership and direction to staff, managing operational issues and ensuring continuous, effective and accountable service delivery.

       Ensure the AN-ACC team understands and works towards achieving contracted performance requirements, including provision and active support of complex AN-ACC assessments on a daily basis.

       Lead workplace education, professional development, quality improvement and reporting activities, including identifying and implementing practice improvement opportunities.

       Liaise with residential aged care facilities, patients, clients and their families regarding assessment logistics, building and maintaining collaborative relationships in line with WSLHD CORE values.

       Manage resources including finances, assets and leave balances in a cost-effective, transparent and accountable manner in accordance with Ministry of Health requirements.

 

This is a maternity relief backfill subject to extension on a part time basis.

Selection Criteria:

1.     Demonstrated extensive specialist knowledge and experience in the area of Aged Care.

2.     Demonstrated management experience in Human Resource Management, Finance, Workplace Health and Safety, Clinical Governance and Quality Improvement and proven ability to lead a team to achieve outcomes.

3.     Demonstrated leadership in effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.

4.     Demonstrated leadership to utilise problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of services.

5.     Demonstrated high level knowledge and understanding of person-centred care, quality improvement and evidence based practice. As well as the ability to effectively prioritise and organise workload of the team.

6.     Demonstrated ability to supervise professional, technical and support staff.

7.     Valid unrestricted drivers licence for use in Australia and willingness to travel in the course of employment.
