About the Opportunity

The Out of Home Care Health Pathway Program is a collaborative initiative between NSW Health and the Dept. of Communities and Justice aimed at improving the health and wellbeing of children and young people in statutory out of home care.

Children and young people in out-of-home-care (OOHC) often have high and unmet health needs and are more disadvantaged and vulnerable than other children. NSW Health provides coordinated health assessments for children and young people aged 0-17 in statutory OOHC living in NSW who are expected to remain in care for longer than 90 days. The pathway ensures children in out of home care receive a Health Management Plan (HMP) that is based on their health and development needs and recommends steps to meet those needs. This plan is monitored and reviewed regularly, where possible children receive priority access to health support and services and their health is supported through changing circumstances.

Expectations of the role:

·         Manage and coordinate referrals to the program and provide consultation and clinical advice in relation to health reviews to all relevant stakeholders who support out of home care health assessments

·         Establish and cement strong and sustainable relationships within CCLHD and the wider health system (particularly GPs & specialist medical providers), key government agencies (especially DCJ, DET) and non-government agencies supporting children and young people in care and their carers

·         Contribute to health assessments of children and young people in OOHC as part of the wider multidisciplinary team context particularly where there is significant complexity

·         Communicate effectively in a culturally sensitive manner with clients, carers and other health workers to plan intervention strategies

·         Provide leadership to a skilled small MDT team and monitor and report on the relevant KPIs for the program to the Ministry of Health

·         Support patient safety, quality improvement, consumer engagement and implementation of clinical guidelines.

About You

We are looking for someone who has:

·         Tertiary qualifications in a health-related discipline and a demonstrated understanding of the health system

·         Excellent negotiation, decision making and advocacy skills and demonstrated ability to work collaboratively with a diverse range of stakeholders

·         Demonstrated experience in either the health or child protection system or both with demonstrated experience of clinical work with vulnerable populations utilising a trauma informed lens

·         Proven leadership capability within a multidisciplinary team environment

·         Strong understanding of the health needs, legislative context and care requirements of children in OOHC

·         Highly developed communication, consultation and negotiation skills with internal and external stakeholders.
