Support older adults experiencing moderate to severe mental health challenges while contributing to recovery-focused care. At Central Coast Local Health District (CCLHD), we are committed to Trusted Care, Better Health for Everyone, and we are seeking a dedicated Older Peoples Mental Health Care Coordinator to join our team.


About the Opportunity

As an Older Peoples Mental Health Care Coordinator, you will provide specialised clinical care and case management for older people in community and residential settings. This role is vital in promoting recovery, wellbeing, and continuity of care for consumers and their families. In this role you will:


• Maintain a caseload and deliver individual, family, and group mental health interventions.
• Develop, implement, and monitor mental health and wellness care plans in partnership with consumers and carers.
• Coordinate access to additional health and support services for older clients as required.
• Collaborate with multidisciplinary team members, providing clinical advice and specialist expertise.
• Contribute to service development, quality improvement, and evidence-based practice initiatives.

This role is suitable for the following:

  • Clinical Nurse Specialist Grade 1
  • Social Worker Level 1 or Level 2
  • Occupational Therapist Level 1 or Level 2
  • Registered Nurse
  • Psychologist
  • Clinical Psychologist

About You

We are looking for someone who has:


• Minimum tertiary qualification in Social Work, Occupational Therapy, or Nursing with Division 1 registration.
• Experience working with older adults experiencing moderate to severe mental health issues.
• Demonstrated ability to work both autonomously and as part of a multidisciplinary team.
• Understanding and application of recovery-focused models in mental health care.
• Strong communication, organisational, and clinical skills to deliver high-quality care and support.
