At Central Coast Local Health District (CCLHD), we are committed to Trusted Care, Better Health for Everyone, and we are looking for dedicated Registered Nurses to join our Integrated Chronic Care team (ICC).


About the Opportunity

As a Registered Nurse in the Integrated Chronic Care team, you will play a vital role in delivering high-quality, community-based care coordination and case management for people living with chronic and complex conditions. Working within a multidisciplinary team, you will support patients to achieve better health outcomes, reduce avoidable hospital admissions, and improve their quality of life.

This position is central to providing holistic, person-centred care in the community. In this role you will:

  • Deliver comprehensive nursing assessments, care planning, implementation, and evaluation using best practice guidelines.
  • Coordinate care for clients with chronic and complex conditions, supporting them to achieve their health and wellbeing goals.
  • Provide expert case management in collaboration with GPs, specialists, and allied health professionals.
  • Support patients and carers through education, advocacy, and promoting self-management strategies.
  • Accurately document and manage healthcare information in line with legal and clinical requirements.
  • Communicate effectively with patients, families, and the multidisciplinary healthcare team to ensure continuity of care.
  • Supervise and support Enrolled Nurses (ENs) as appropriate.

You will be part of a broader Community Chronic and Complex Care team, contributing to key initiatives such as Planned Care for Better Health and integrated care programs aimed at improving outcomes for the Central Coast community.

About You

We are looking for someone who has:

  • Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Recent clinical experience in chronic and complex care, community health, or a related specialty.
  • Strong verbal, written, and interpersonal communication skills, including the use of clinical information systems.
  • Demonstrated ability to work effectively within a multidisciplinary team environment.
  • Excellent organisational skills with the ability to prioritise workload and manage competing demands.
  • A commitment to evidence-based practice, quality improvement, and ongoing professional development.
