What you'll be doing
The Western Sydney HealthPathways program is a strong clinically based partnership comprising the Western Sydney Primary Health Network (PHN), the Western Sydney Local Health District (WSLHD) and the Sydney Children’s Hospitals Network (SCHN). This partnership delivers the HealthPathways program which is a key enabler for supporting the integration of care across the primary, secondary and tertiary care sectors, as well as supporting the aim of providing patients with “the right care, in the right place, at the right time” with easily accessible online tools to help assess, diagnose and manage a broad range of conditions and symptoms, with appropriate referral of patients where and when required.
The program aims to achieve and support:
This role will lead the WSLHD efforts in the collaborative development and coordination of a partnership program (Western Sydney HealthPathways) driving evidence based care pathways as a key enabler for integrated care accessible to GPs and other healthcare professionals and sector providers.

What you'll be doing

This partnership delivers the HealthPathways program which is a key enabler for supporting the integration of care across the primary, secondary and tertiary care sectors, as well as supporting the aim of providing patients with “the right care, in the right place, at the right time” with easily accessible online tools to help assess, diagnose and manage a broad range of conditions and symptoms, with appropriate referral of patients where and when required.
The program aims to achieve and support:
- Integrated care and services across primary, secondary and tertiary sectors and with key partners
- Improved patient and provider experience and access in the health system
- Reduced time for patients and providers as they navigate the system
- Improved health outcomes for patients and better quality of life
- Reduced avoidable ED presentations and/or potentially preventable hospitalisations
- Decreased clinical variation and better patient safety at a local system level
This role will lead the WSLHD efforts in the collaborative development and coordination of a partnership program (Western Sydney HealthPathways) driving evidence based care pathways as a key enabler for integrated care accessible to GPs and other healthcare professionals and sector providers.
- Relevant qualifications, experience and/or training in a health related discipline, as well as project management or working in project teams
- Ability to lead and influence others and develop and maintain partnerships internally and with external organisations, with demonstrated experience working effectively in a partnership based team environment
- Strong strategic and process management skills to provide leadership on the delivery of the HealthPathway program within WSLHD
- Possess strong organisational skills required to identify strategic priorities, lead and manage workflow, complete or follow up necessary tasks
- Excellent written and verbal communication demonstrated recently with senior Executives, senior health professionals, managers and consumers
- Excellent interpersonal and negotiation skills to engage and form productive partnerships at a senior level with a broad range of internal and external stakeholders
- Ability to write detailed, strategic reports, documents and correspondence including use of medical/health management terminology to deliver complex correspondence for Executive and Partnership managers, and other senior staff.
- Demonstrated experience in utilising a wide range of IT programs, in particular Microsoft office standard package (Excel, Outlook, PowerPoint and Word) and current IT systems within NSW Health
