About the Opportunity

As the Transitional Nurse Practitioner (TNP)– Renal, you’ll play a pivotal role in delivering innovative, patient-centred care for people living with chronic kidney disease (CKD) across the Murrumbidgee Local Health District (MLHD).
Working both autonomously and collaboratively, you’ll support consumers throughout the renal care continuum—from early CKD screening and education to advanced symptom management and supportive care.
This role spans regional facilities, nurse-led clinics, and community settings, with a focus on enhancing quality of life, reducing disease burden, and improving access to timely specialist care.

As a TNP-Renal, you’ll function within an advanced clinical scope—providing comprehensive assessments, prescribing within your authorisation, initiating referrals, and ordering diagnostics to ensure integrated, evidence-based management.

Your leadership will also extend to education, research, and service development, championing innovation and best practice in renal care delivery across rural and regional communities.

About You

  • You value innovation, evidence-based practice, and continuous improvement—and you’re not afraid to challenge traditional models to create better outcomes for patients and families.

You are:

  • Experienced in renal nursing and chronic disease management, wanting to expand your career as a TNP.
  • Autonomous yet collaborative, confident working within and leading multidisciplinary teams
  • Analytically minded, using data and evidence to inform clinical decisions
