About the role
As a Registered Nurse – Acute Stroke Care Coordinator, you’ll coordinate the care of patients presenting with acute stroke, ensuring rapid assessment, timely treatment and seamless multidisciplinary collaboration from arrival through to discharge planning. You’ll take the lead in coordinating acute stroke pathways and responding to stroke alerts, facilitating urgent diagnostics and treatment, and working closely with Emergency, medical, nursing, radiology and allied health teams to keep care moving efficiently.
You’ll monitor key stroke performance measures, contribute to quality improvement, data reporting and education, and act as a clinical resource and advocate for stroke‑affected patients and their families. Reporting to the Deputy Director of Nursing (DDON), the role has no direct reports but carries strong clinical leadership expectations, including supervising and mentoring staff when required, delegating appropriately to Enrolled Nurses (ENs) and Assistants in Nursing (AINs) and responding effectively to changing patient status and emergency situations. Together, these responsibilities allow you to deliver safe, compassionate, person‑centred stroke care in line with evidence, NSW Health values and organisational standards.
About you
You’re a self‑motivated, proactive Registered Nurse (RN) with a strong focus on improving patient outcomes and the confidence to lead in time‑critical situations. You work effectively within an interdisciplinary team, communicate clearly with patients, families and colleagues, and act as a calm, reliable presence when clinical priorities are competing.
You bring:
- Current Authority to Practice as a Registered Nurse with AHPRA
- Demonstrated ability to work within an interdisciplinary team and build collaborative relationships
- Strong time management skills and ability to prioritise competing workloads in a complex environment
- Demonstrated use of evidence-based practice and critical thinking in delivering patient‑centred care
